RELOCATION ASSISTANCE Assistance finding appropriate living situation whether it's at home, independent living, assisted living or nursing facilities. PATIENT ADVOCACY Acting on behalf of the individual rights, wants and needs. The Care Manager is the eyes, ears and voice of the aging adult. INFORMATION AND ASSISTANCE The knowledge of community resources and services allows the Care Manager to be a single point of access for all needs and concerns of an aging adult and the loved ones. CRISIS INTERVENTION Identifying immediate needs and providing access to benefits, resources and professionals to get the indivudal back on track. ON-GOING CARE MANAGEMENT The Care Manager will continually monitor the individual. As their needs may change due to decline in health, change in status or other, the Care Manager will adjust the care plan to ensure they have the best possible care available and their needs and goals are being met. INDIVIDUALIZED CARE PLANNING Assessing current need as well as short and long term goals basedon physical, social, emotional, legal and financial factors
